The most important thing in the syringe programs is their preventive goal which materializes in reducing the risks of IV drug users (IDUs) form getting infected with HIV/ AIDS and other sexually transmitted infections (STIs) like Hepatitis B and C. These strategies are considered basic in harm reduction programs of addiction to prevent IDUs from getting infected with STIs. The Syringe programs comprise of different sorts of services; like providing sterilized injecting materials, educating people about reduction of addiction, providing health information, orienting people to seek health care centers and providing medical and legal services in order not to provide drugs for addicted people.

A hypodermic needle-exchange program is a sometimes controversial social policy, based on the philosophy of harm reduction where injection drug users can obtain hypodermic needles and associated injection equipment at little or no cost. These programs are called "exchanges" because many require exchanging used needles for an equal number of new needles. In practice, some programs vary in their stringency; in the Canadian capital Ottawa, for example, participating clinics do not demand used needles before giving out new ones. By Jason Farrell Build it and they will come. Well we built it and they are coming at an average of 50 new injectors a month, 600 a year and about 100 a day walk thru our doors. To say that Positive Health Project, Inc. (PHP) is just a syringe exchange program is basically belittling who and what we are and what we do. As AIDS prevention programs sprouted up throughout the country during the early days of the epidemic, they were primarily designed for gay men and for prevention issues for gay men. Surprisingly at that time all HIV pre and post test counseling prevention information for gay men did not include drug risk reduction messages. HIV and HCV prevention information for drug users was not included or available until the late 80s, early 90s when syringe exchange programs gained attention. This was largely due to the fact that injectors nationwide were becoming infected at high rates and syringe exchange was and even today is illegal in many cities and states. CDC and health department recommendations during this period were to promote the use of bleach kits and latex barriers for safer sex practices. All of which seemed like a good option, but unfortunately a significant number of states and county health departments did not allow bleach distribution, and in some counties possession of more than one condom could result in arrest for or promoting prostitution.
